Consents and Declaration
How we collect, hold, use and disclose information about you is important to us. The purpose of collection, holding, use and disclosure of the information is to fulfil the ministry and administrative functions of the QB YA Retreat 2021. We also collect your information to invite you to any future events. If you do not provide your information, we may not be able to do this. We may communicate with you for these purposes by email or mail, which you can unsubscribe from at any time by writing to us. We may share our information collected with other Baptist Churches, affiliated organisations and hosts of events you are attending. Media & Photography Consent
As part of attendance at the QB YA Retreat, I give permission for my photo/video to be taken and stored by QB Youth & YA Ministry of Baptist Union of Qld (in accordance with the Privacy Act of QLD).
Video footage and photographs will be taken to record activities and may be used to promote the QB Youth & YA and its various Youth & YA activities in both print and digital formats, including websites, social media and other digital forms of communication.
You may withdraw this permission at any time by writing a letter to QB Youth & Young Adult Ministry, PO Box 6166, MITCHELTON QLD 4053.

I confirm I will not attend the QB YA Retreat if I:
Am experiencing cold or flu-like symptoms;
Have a temperature;
Have, in the 14 days prior to Retreat, been in contact with someone that is
to have COVID-19;
Have, in the 14 days prior to Retreat, returned from overseas or an Interstate Hotspot.
Have been contacted as part of Queensland Health Contact Tracing efforts in the previous 48 hours prior to Retreat and not been tested for COVID - 19.
Are awaiting the result of a COVID - 19 Test.
I understand that between the date of signature and the first day of Retreat I must inform the QB Youth and Young Adults Ministry Leader if I come into contact with someone that is confirmed to have COVID-19.
I understand temperatures may be monitored throughout Retreat by touchless thermometers.
I understand that if there are changes in restrictions, QB reserves the right to cancel the Retreat, in which case you will receive a full refund or be given the option to transfer your money to another booking. NB: Registered attendees will not be penalised for not attending when unwell and will be encouraged to stay at home and not attend the event.
I understand if I experience cold or flu-like symptoms or an elevated temperature during the event or are contacted as part of Queensland Health Contact Tracing arrangements, I will be moved to a quarantine area and arrangements will be made for me to return home or to appropriate medical facilities.

COVID Safe Event Plan Information
I understand that there will be a COVID Safe Event Plan in place for YA Retreat 2021, which I agree to abide by. This includes such things as:
Stay at home if unwell.
Seek assistance from QB Youth and Youngs Adults Ministry Leader if becoming unwell during the event.
Awareness of first aid locations.
Follow the enhanced public health measures that are in place at the event.
Downloading of the Commonwealth Government COVID-19 app prior to attending the event is optional.
Respond to and follow announcements/directions provided during the event.
Follow good hygiene practices, observe occupant density limits, and maintain physical distancing requirements at all times.
The Baptist Union of Queensland, and its subsidiaries, including Queensland Conference and Camping Centres (QCCC) disclaim liability for any and all loss, damage, injury or illness, financial or otherwise, suffered by any person in the attendance of the Queensland Baptists Retreat, use of the QCCC Noosa Conference and Camping Centre and its facilities, and on-site activities and programs, except in the event that the loss or damage, illness or injury is caused by the negligence of the Owner/Operator, Manager or staff of Queensland Conference and Camping Centres.
